All Guests must fill out the Guest Liability Waiver prior to entering the Club. Guests under the age of 18 must have a parent or legal guardian complete this form.
A Non-Parent cannot legally sign this waiver for other’s people’s children.
The waiver is valid for the entire summer and on needs to be filled out once. Guests are permitted to visit the club 3 times per calendar month.
